To participate in camp it is required each camper print the below applicable Forms and bring them with you to camp! These forms can be turned in at the registration table the day of the camp.
The Release Form must be signed by a parent or guardian if student is under 18 years old. Students who are 18 years old or will become 18 years old before the end of the camp / clinic must also sign. Otherwise, parent or guardian must be contacted prior to release to participate.
If you register online, you will have the required forms e-mailed to you.
